Registering a quilt is simple. To request an inventory form, simply email (quilts@museum.msu.edu) or call the Great Lakes Quilt Center (517/432-3800) or print a form here. Fill out the form, attach a photograph of the quilt and if possible, of the quilter. Include any ephemera and mail to:

Once the form is returned to the GLQC it will be assigned a number and the data will be entered into a computerized system. You will be sent an acknowledgement email with your quilt’s assigned number. MQP identification packets, available for $15.00 per quilt from the GLQC/MSUM; include a quilt report with the MQP photograph of your quilt, a quiltmaker's report, and a fabric quilt label printed with the quilt’s MQP number and title that can be sewn on to the back of your quilt.
